They fill out the physician section including their Florida medical license number, diagnosis, and signature. Most PCPs charge $75-200+ for this type of office visit.

Complete the Patient Section

Fill out your personal information, Florida driver’s license number, vehicle details (VIN, make, model, year), and specify which windows require exemption. All information must match your Florida vehicle registration.

Submit to FLHSMV

Mail or deliver the completed form to FLHSMV in Tallahassee or your local tax collector office. Processing can take several weeks. Once approved, you’ll receive documentation, if independently issued after review and information about obtaining your required compliance sticker.
