Florida Tint Exemption("we," "us," or "our") provides an online service that helps Florida drivers apply for a window tint medical exemption. This site and MyEyeRx coordinate intake, referrals, and administration. Any clinical review is performed by an independent, appropriately licensed clinician who makes an individual decision. We take the privacy and security of your information seriously, especially your health information. This policy applies to floridatintexemption.com and our online application.
Information We Collect
- Identifying information: name, date of birth, residential address, email address, phone number, and the state in which you drive.
- Health-related information: the medical condition(s) you report, symptom duration and frequency, medications, prior care history, and any medical documentation you choose to upload.
- Payment information: your card details are entered into a secure, PCI-compliant checkout hosted by our payment processor (Stripe) and tokenized. We never receive or store your full card number.
- Technical information: limited data such as IP address and device/browser details, used for security and to operate the website.

- To process the intake and, when applicable, facilitate review by an independent, appropriately licensed clinician.
- To coordinate follow-up and provide documentation when independently issued after review.
- To process payment for the consultation.
- To communicate with you about your application, including requesting additional documentation.
- To comply with legal, regulatory, and recordkeeping obligations.
We follow data-minimization principles and access or use only the information reasonably needed for these purposes.
How We Share Your Information
We do not sell your personal or health information. We share information only as needed to provide the service:
- Independent, appropriately licensed clinicians when a case-specific clinical review is requested.
- Service providers, including our records platform (GoHighLevel) and our payment processor (Stripe, which handles payment information but is not sent your medical-condition response by this website).
- OpenAI for the optional automated support chat. Text typed into that chat may be processed under the applicable OpenAI service terms and account data controls. Do not enter medical records, dates of birth, addresses, payment details, or other sensitive personal information in chat.
- When required by law, such as a valid legal request or to protect safety.
Health Information Privacy
MyEyeRx coordinates intake, referral, and administration; this site does not make clinical decisions. When information is protected health information held by a covered healthcare provider, federal and state health-privacy requirements may apply in addition to this policy. Your Health-Information Rights

- Access controls within the records platform used by the coordination team.
- Tokenized, PCI-compliant payment handling — card data never touches our servers.
